Bloodhunt gives you a bonus to your attack, but you seem to be missing something, probably your bonus to attack from CA. You should have a total of +11 to attack:
+3 DEX
+3 Dagger proficiency
+1 Rogue Weapon Talent
+1 Bloodhunt
+1 Enhancement
+2 CA (enemy can't see you)

When Rogues can maintain combat advantage, they can be very accurate attackers indeed.

For Sly Flourish, you add your Charisma bonus to damage, so you should indeed be dealing 1d4+6 damage:
+3 DEX
+2 CHA
+1 Enhancement

So that's the correct damage and you hit anyway but remember all your bonuses (and it helps to list them out once in a while to make sure you've covered everything you deserve.)
