Your Stat Block should include the following in addition to whatever else you'd like to have on there.
Weapons you wield including Attack and Dmg Modifiers and/or Prepared Spells with spell slot availability and use (0/4, for example)
Any continuous effect that your character may be under. (Mage Armor, etc) Basically anything that you want me to remember is active on a normal basis even out of combat.
AC, HP, Saving Throw Modifiers, Resistances/Immunities/Vulnerabilities, Your vision type (Darkvision, etc), Your Speed and languages.
Here is an Example of what I usually use. Your encounters will have a similar stat chart during combat for the Party and Enemy. You are more then welcome to use this, as I've stolen it and tweaked it to my own liking a million times. It's heavily coded but I only change it rarely (rests, levels, etc.)

Appearance: A young tiefling in her early twenties. Her skin, hair, and eyes all have a deep purple tint. Her features are pretty but hard. Her face naturally rests in a glower that tends to put off others even more than her horns. Her smile is rare but incredibly striking. She stands straight, though her movements and stature are flexible and lithe.
Personality: Cautious. Selfish. Defensive. Vera doesn't let her guard down. She expects others to pull their weight and will pull hers as well but won't pull it first without knowing she isn't being taken advantage of. She generally assumes anyone she meets is looking to steal from her or worse. Even those she has known for a long time she will keep at an arm's length. Her words, the few that she speaks, are calculated and intentional. Her sense of humor is dark, but she can have a few bright moments. Occasionally she may even laugh, though that will usually be at the expense of other people. While she may not be the most outgoing girl, she can be charming enough when the need suits her.
Trait: I think anyone who's nice to me is hiding evil intent. Trait: I am utterly serene, even in the face of disaster. Flaw: I believe that everyone is beneath me.
Something changed. Something felt... wrong... different... Light! There was light! Vera's eyes flung open. Just as quickly, she shut them tight and reached both hands up, pressing palms against her eyelids. Her eyes stung against the bright light, like she hadn't used them in... she couldn't remember. She couldn't remember anything other than the feeling that there had been a fleeting moment just on the other side of the light. What it was... she wasn't sure.
Slowly she lifted her palms from her eyes and squinted them open slowly. She had no idea where she was. She was in a bed, that was a start. She was wearing clothes, but they felt wrong. They were loose, baggy. She stood, her legs wobbled underneath her for a moment before she adjusted. Movements took effort. Her arms felt heavy, her legs didn't respond, her head weighed on her neck. Regardless, her mind was sharp. She took in the room through eyes that were slowly adjusting to the assaultive brightness. Four other beings were there, sleeping, as she must have been moments before.
Vera slowly made her way to a darker corner of the room, using the furniture for support along the way. She leaned her back against the wall and allowed her eyes to fully adjust. It took a surprising amount of time, and in that time she assessed the situation. She had no clue where she was or how she had gotten there... she still knew who she was... kind of... she was Vera, and that was a start at least.
