Asking as someone who is not too familar with the setting of Greyhawk, but willing to learn: What era or year in this campaign set it? Seems like the setting changed quite a bit over the years...

That it did. This was Gygax's baby, so everything he and his friends did in this world is reflected in the timeline, which then in turn shaped how Greyhawk came to be. It also happens to be where the vast majority of 1E adventures were supposed to take place, even if they weren't specifically stated where in Greyhawk. As an example, the Isle of Dread (X1, from the Expert Set) is south of the Amedio Jungle as part of the Isles of Axuxal. So a lot of official and tournament/competition adventures that took place shaped Greyhawk as well.

To put a fine point on the timeline, though, we are in the Common Era, which spans from 1 CY to 598 CY. To further this, we are in the Year of Peace, 585 CY. This is after the Pact of Greyhawk was signed, which brought an end to what will eventually be known as the Greyhawk Wars. We are going to be early in the year, so if you go by the canon timeline, the incident in Needfest hasn't happened just yet...but it's about to.

Question: Can someone who is grappled be used as an improvised weapon? Like grab someone by the ankle and swing them around like a flail?

I think one of the new giant themed subclasses does that? Barbarian I think. Specifically mentions throwing creatures like projectiles lol.

Yep. Roll20, which is basically lifted directly from the books, has this quote at https://roll20.net/compendium/dnd5e/Weapons#toc_14:

Improvised Weapons
Sometimes characters don’t have their weapons and have to attack with whatever is at hand. An improvised weapon includes any object you can wield in one or two hands, such as broken glass, a table leg, a frying pan, a wagon wheel, or a dead goblin.

Although the reference states a dead enemy, this could be applied to any living creature you are grappled with. Without looking up specific rules, my guess is you'd have to make a STR check to remain grappled, then an attack roll - probably with disadvantage, and without your proficiency bonus unless you have a feat or ability that allows otherwise - to attack with said flailing grapplee.
